Bangladesh: Requirements for replacing a lost Bangladeshi passport in Ottawa, including identity documents required and whether physical presence of the applicant is required

The information that follows, published by the Bangladesh High Commission in Ottawa, updates the information provided in BGD30033.E of 17 September 1998:

1.Attached "KA" form to be filled and signed by the applicant at both places on the back page of the form and sent back to us with applicant's old passport. Four (4) recent colour passport size photographs are needed. If the passport is more than twelve (12) years old, six (6) copies photographs are required.

2.Child Endorsement: (up to twelve years age only)

a. If child is already endorsed in his/her mother's old passport: 2 copies of stamp size (2.5cm X 2.6cm) photographs are needed.

b. If not endorsed before: 2 copies of stamp size (2.5cm X 2.6cm) photograph needed.

c. Birth certificate where both mother and father's names are mentioned.

d. A statement of facts on plain paper.

e. Physical appearance of the child is required.

3.In the case of a lost passport: six (6) recent coloured passport size photographs, a police report, birth certificate, academic certificate and a statement of facts on plain paper is required. The applicant must come to the High Commission to apply for a new passport. 'Ka' form has to be filled by the applicant.

4.Physical presence of the applicant is required for issuance of fresh passports in the case of lost passport (Bangladesh n.d.).
